Emergency room diversion programs
Partner members, established a pilot Emergency Diversion program between the Miami-Dade Health Department and Jackson Health Systems in an effort to increase Emergency Room (ER) efficiency by diverting patients seeking non-emergency care in Jackson Hospital’s ER to a primary care setting. This program has been active since January 2009 and forwards a county-wide referral system to connect the uninsured to the provision of continual, managed, and coordinated care through a medical home.
Point of Care Electronic Health Records Initiative
Provided the platform for partner members to secure funding for the establishment of an electronic health record system in two free clinics in Miami-Dade County. This program allows for the expansion and dissemination of a successful, multi-market, sustainable business model for the adoption of Electronic Health Records (EHR). In addition, this program harnesses the power of EHR to improve the delivery of primary care for the uninsured population, and thereby improving health outcomes and reducing community-wide health costs.
South Florida Health and Human Services Application for Resources and Eligibility (SHARE) Miami, common eligibility

Lead efforts to establish the SHARE Miami project. Utilizing the One-e-App tool, the SHARE Miami project will implement an innovative web-based system for connecting families with a range of publicly funded health and human service programs, through a one-stop approach to the application process. The project is currently under development with an expected launch date of spring of 2012.
South Florida Regional Extension Centers http://www.southfloridarec.org/
MD-HAN served as a partner for the procurement of a $13 million grant from the Department of Health and Human Services, Office of the National Coordinator for Health Information Technology (ONC), to establish a Health Information Technology Regional Extension Center in South Florida. The center will provide technical assistance, guidance, and information on best practices, as well as support and accelerate health care professionals’ efforts to become meaningful users of Electronic Health Records (EHRs).
