HEALTH CARE ACCESS THROUGH COMMUNICATION - COLLABORATION - COORDINATION AND COOPERATION

Background
In the spring of 2008, the Miami-Dade County Health Department (MDCHD), in partnership with the Health  Council of South Florida and Florida State Representative Juan Carlos Zapata, convened a number of community stakeholders to  address the issue of healthcare access for the uninsured. As a result of this meeting, the Miami-Dade Health Action Network (MD-HAN) was formed in May 2008 with the nomination of six Steering Committee members and three Ad Hoc Members.

Vision
To realize an integrated safety-net that cooperatively maximizes its resources to provide care and leverage policy.

Mission
To serve as a catalyst and resource for safety-net innovation, coordination, collaboration, and policy direction that promotes accessible and affordable health care to the uninsured and underserved of Miami-Dade County.
