The Health Council of South Florida (HCSF) has established an extensive public-private collaborative network of health service providers, payers, community organizations, and policymakers to improve county-wide health by collectively tackling challenges and maximizing opportunities resulting from healthcare reform. The network, called The Miami-Dade Health Action Network (M-DHAN), is a neutral convening group to develop and implement joint, integrated health strategies in Miami-Dade County through aligned resources, priorities, and measures of success. The M-DHAN consists of 436 community organizations representing virtually all of the county's 2.7+ million residents. The community organizations include hospitals, federally qualified health centers (FQHCs), behavioral health providers, public health groups, local government officials, regional leaders, academics, social service providers, urban planners, insurers, business leaders, community based non-profits, and faith-based leaders.

The M-DHAN Steering Committee is the advisory group responsible for guiding the operations of the Network and the achievement of the M-DHAN’s vision and mission by providing actionable recommendations to the Transformational Leadership Council (MD-TLC). The MD-TLC composed of a cadre of key legislators and local decision-makers who are able to affect change. In further support of the M-DHAN’s vision and mission, the following four work groups work actively on: Data/Technology, Primary Care Safety-Net, Community Health Worker/Patient Navigator and Innovation/Impact.
